A “no show” for a scheduled appointment will be counted as a lesson. I agree, understand and acknowledge that the elimination or modification of behaviors is not guaranteed. The client acknowledges that dog training will not provide exact results as each dog is different in regard to ability, breeding and temperament. Client further agrees to accept responsibility for any damages the above-named dog may cause through malicious, aggressive or improper behavior that may occur before, during or after all services provided by Canine Cornerstone, LLC. I agree and understand that my participation and/or involvement in dog training carries with it the potential for certain risks, some of which may not be reasonably foreseeable. I assume all risks associated with participating in this training and will not hold Canine Cornerstone, LLC responsible in the event of injury to Client, client’s family member or Dog. I agree to comply with the instructions, rules and decisions of the training instructor as it relates to Clients, or dog’s ability to safely complete each training session. I also agree to assume all responsibility for any damage done to property, persons, or other dogs done by Client or Dog’s actions. I agree to release, indemnify, and hold harmless Canine Cornerstone, LLC and it’s agents and affiliates from all losses, claims, demands, liabilities, causes of action or expenses, known or unknown, arising out of my participation in dog training sessions with Canine Cornerstone, LLC. I agree and understand that the above named dog participating in the training sessions is free of any infectious disease and is current on all appropriate vaccinations, including bordatella, distemper, parvovirus and rabies. Checking this box indicates that I understand Canine Cornerstone, LLC can use my pet’s photograph(s) and/or video(s) taken during training sessions for educational and promotional purposes in any type of media. Governing Law {i} Mediation. If any claim, controversy or dispute arises out of or relates to this Dog Training Agreement and if the dispute cannot be settled through negotiation, the parties agree first to try in good faith to settle the dispute by mediation administered by the American Arbitration Association under its then mediation rules. {ii} Arbitration. If the parties are unable to settle the dispute after mediation, then the dispute shall be settled by binding arbitration administered by the American Arbitration Association, or such other similar dispute-resolution organization as the parties may so agree. {iii} This Dog Training Agreement shall be governed by the laws of the state of Washington and mediation or arbitration in Seattle, Washington. {iv} Prevailing party shall NOT be entitled to attorney, mediation or arbitration fees. Terms & Conditions Canine Cornerstone agrees to provide the following services: Private Training Sessions – Customized session to tailor your dog’s specific needs. Initial consultation: $210 for up to 120 minutes (one-time only will add 30 minutes at no charge).
